Your New Care

We very much look forward to providing you with high level care. We appreciate the opportunity to do so. We understand that a practice transition can be very stressful for patients, change is always challenging, when it comes to our health care it is particularly challenging. This is compounded by the COVID-19 pandemic which has made health care access in general very challenging for patients.

We appreciate that this may present some significant challenges for patients attempting to access things such as prescription renewals, completion of forms that may be necessary to receive required funds and even care for acute medical concerns. We are aware of this and we will make every attempt possible to ensure a smooth transition through the practice transfer as well as through the remainder of the COVID-19 pandemic. Please read on below for an outline of the accommodations that we are making to facilitate a smooth transition of care and to account for the fact that this is occurring amidst the COVID-19 pandemic. This approach is distinct from our standard approach to providing you care, which we will employ once the restrictions on in person care have been lifted, these practice policies can be found here.

Accessing Your Medical Records

As noted in the letter to all patients, we do not have a copy of your records, these records have been assumed by Docudavit. Contact information for Docudavit can be found here.

Changes To Standard Procedures

It is important to note that things that are typically viewed as standard, or routine may not at all be routine during this time. An example is renewal of prescriptions. We do not have your medication profile, and in order to renew most prescriptions we will require it. This will need to be provided by your pharmacy on most instances in order to proceed. We will work with you and your pharmacy to ensure that all the necessary information is received, and that there is no gap in your access to medications. We ask that you work patiently with us, as we are very much doing our best to serve you through this difficult time in health care. Another example is the completion of a form for insurance. Many of these forms require that the healthcare provider completing it have access to your medical records, in order to verify the statements made on the form. Some of these forms often seem trivial to patients, but they are unfortunately not, and cannot be completed without access to medical information. In these instances, we will work with you to facilitate the transfer of your chart, which will allow us to complete your form as quickly as possible.

 

New patients:

We welcome all new patients to our practice. We do not have an interview process; we accept new patients in the order in which they contact us.
